A new scientific statement from the American Heart Association, published in Circulation, concludes that consuming up to 400 mg of caffeine per day—equivalent to about five 8-ounce cups of caffeinated coffee—is safe for most adults and may be associated with a lower risk of cardiovascular disease. However, higher doses of caffeine, particularly from energy drinks and shots, can increase risks of high blood pressure and irregular heart rhythms.
The statement, titled “Caffeine and Cardiovascular Disease,” reviewed the latest research on caffeine’s cardiovascular effects. Chair of the writing group, Dr. Gregory M. Marcus, noted that while moderate coffee intake is safe and potentially beneficial, high-dose caffeine sources like energy drinks should be avoided. The statement emphasizes that coffee contains bioactive compounds with antioxidant and anti-inflammatory properties, which may contribute to health benefits independent of caffeine.
Key findings include: consumption of 2–4 cups of caffeinated coffee daily is linked to lower risks of heart disease, heart failure, and stroke; 1–3 cups daily is not associated with increased atrial fibrillation risk but may increase premature ventricular contractions; and unfiltered coffee (e.g., espresso, French press) contains cafestol, which can raise LDL cholesterol. Adding sugar, cream, or syrups likely reduces health benefits.
The statement also highlights that responses to caffeine vary based on genetics, metabolism, age, and medications. Short-term effects can include temporary increases in blood pressure and heart rate. The writing group calls for more randomized controlled trials, especially on energy drinks and other caffeinated products, to better understand their cardiovascular impacts.
Dr. Marcus stressed that there is no one-size-fits-all approach: “What may be a reasonable amount for one person could cause unwanted effects in another.” The statement does not provide specific treatment recommendations but informs future dietary guidelines. The AHA’s 2026 Dietary Guidance to Improve Cardiovascular Health did not reference caffeine due to insufficient data for formal recommendations.
