Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded collaboratively by Pfizer Inc. and the National Comprehensive Cancer Network (NCCN). The grant will support a collaborative project to deploy Avident Health's proprietary MyNavigator digital platform to enhance care delivery for patients diagnosed with metastatic breast cancer. This initiative matters because metastatic breast cancer requires complex, ongoing management, yet patients and caregivers often struggle to navigate clinical guidelines and access supportive resources between clinic visits. By bridging these gaps, the project aims to improve treatment adherence, shared decision-making, and quality of life.
The MyNavigator platform will serve as an interactive, home-based companion that delivers tailored education aligned with NCCN Guidelines for Patients. It translates complex clinical pathways into accessible, bite-sized modules tailored to each patient's specific treatment journey, empowering patients to participate actively in informed clinical decision-making. The platform also directly integrates Unite for HER's comprehensive suite of free services—including professional nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—into daily navigation prompts. This streamlined access removes barriers to integrative supportive care.
Uniquely, the platform engages informal family caregivers by including them in a secure, proactive communication loop. This ensures they have the context, education, and early symptom alerting needed to support treatment adherence at home. Additionally, the platform captures patient-reported check-ins and symptom trends to alert clinical teams early, preventing avoidable complications and reducing acute care utilization. By equipping caregivers and enabling proactive interventions, the initiative fosters a continuous circle of support.
Dr. Maen Farha of Avident Health stated, "Navigating metastatic breast cancer is a daily reality that extends far beyond the four walls of the clinic. We are honored to receive this support from Pfizer and NCCN. By combining the proactive digital coordination of MyNavigator with Unite for HER's exceptional integrative services and NCCN's gold-standard guidelines, we are creating a continuous circle of support that empowers patients, supports clinical teams, and actively engages the family caregivers who are vital to treatment success."
Sue Weldon, Founder and CEO of Unite for HER, added, "Unite for HER is dedicated to provide a supportive community and transformative integrative care directly to metastatic breast cancer patients. Collaborating with Avident Health on the MyNavigator platform enables us to reach patients and their families directly at the exact moment they need guidance, removing barriers to our integrative nutrition, wellness, and emotional support resources."
The project will measure key quality improvement endpoints over the grant period, evaluating improvements in patient knowledge concordance with NCCN Guidelines, rates of supportive service utilization, patient-reported decisional conflict, and therapy adherence metrics.
