BioVersys BV100 Data in Leading Journals Strengthen Case for Phase 3 CRAB Pneumonia Trial

BioVersys published Phase 1 data showing BV100 achieves high lung penetration and has a milder drug-interaction profile than rifampicin, bolstering the pharmacological rationale for its ongoing Phase 3 RIV-TARGET trial in carbapenem-resistant Acinetobacter baumannii pneumonia.

BioVersys BV100 Data in Leading Journals Strengthen Case for Phase 3 CRAB Pneumonia Trial

BioVersys AG announced the publication of two Phase 1 clinical pharmacology studies of its lead asset BV100 in the International Journal of Antimicrobial Agents and the Journal of Antimicrobial Chemotherapy. The data are significant because they address two critical questions for an intravenous antibiotic intended for critically ill pneumonia patients: whether the drug reaches the site of infection in the lungs and whether it can be safely co-administered with the many other medicines such patients receive.

The lung penetration study, published in the International Journal of Antimicrobial Agents (DOI: 10.1016/j.ijantimicag.2026.108014), enrolled 36 healthy male participants who received BV100 (intravenous rifabutin) 300 mg every 12 hours for four days. Concentrations of rifabutin in lung fluid and alveolar macrophages exceeded plasma concentrations at every time point, with median penetration ratios of approximately 35-fold and over 300-fold, respectively. Repeated dosing was well tolerated. The authors conclude that these pulmonary exposures provide a strong pharmacological rationale for further clinical evaluation of BV100 in severe pneumonia caused by carbapenem-resistant Acinetobacter baumannii-calcoaceticus complex (CRABC).

The drug-drug interaction studies, published in the Journal of Antimicrobial Chemotherapy (DOI: 10.1093/jac/dkag317), show that BV100 is a mild CYP3A inducer, reducing midazolam exposure by approximately 47%, and a mild substrate for CYP3A inhibitors, with rifabutin exposure increasing 1.3- to 1.7-fold with itraconazole. This is a milder interaction profile than that of rifampicin, a strong CYP3A inducer. The implications are clinically meaningful: critically ill patients often receive multiple medications, and a milder interaction profile reduces the risk of unpredictable drug levels and supports the use of BV100 alongside standard care.

Both datasets form part of the clinical pharmacology foundation of RIV-TARGET, the ongoing Phase 3 registration trial of BV100 in hospital- and ventilator-associated bacterial pneumonia caused by CRABC. Enrollment is ongoing, with topline data anticipated in early 2028. BV100 is also being studied in the open-label Phase 2b RIV-CARE differentiation trial in collaboration with ADVANCE-ID.

Dr. Glenn Dale, Chief Development Officer of BioVersys and co-author of the publications, said the papers address the questions that matter most for an intravenous antibiotic in critically ill pneumonia patients, noting that BV100 has very good distribution in lung fluid and alveolar macrophages and a mild, manageable drug-interaction profile. Dr. Marc Gitzinger, Chief Executive Officer and founder, said carbapenem-resistant Acinetobacter baumannii represents one of the highest medical needs in infectious disease, and these data strengthen the scientific basis for BV100 therapy in CRABC pneumonia.

The publications come as BioVersys seeks to differentiate BV100 in a field with few options. The company forecasts that annual carbapenem-resistant A. baumannii infections in hospitals have surpassed one million globally, with mortality rates up to 50% due to limited treatment options. BV100 received Fast Track Designation from the US FDA in September 2026 and Qualified Infectious Disease Product designation in 2019. The new data are important because they provide peer-reviewed evidence that BV100 reaches the lung at high concentrations and can be used with other drugs, potentially supporting its eventual use in a patient population with urgent unmet need.
