Four Medical Societies Unite to Redefine Women's Heart Health Across the Lifespan

The American Heart Association, American Academy of Pediatrics, American College of Obstetricians and Gynecologists and the Society for Maternal-Fetal Medicine have jointly published a landmark three-part framework that redefines cardiovascular health for girls and women. The guidance, released in Circulation, establishes four distinct “Female Life Stages” and calls for a national network of Comprehensive Centers of Excellence for Women’s Cardiovascular Care. The effort matters because cardiovascular disease remains the leading cause of death in women, claiming more than 400,000 U.S. lives annually, yet only 41% of women recognize it as their greatest health threat. The multi-society approach aims to close that gap by giving clinicians across specialties a shared foundation for recognizing risk from puberty through menopause and beyond.

The new framework is built on female biology, dividing life into perimenarche (ages 8-15), ovarian cycling (15-44), perimenopause (45-51) and postmenopause (51 and older), with pregnancy recognized as a biological “stress test” that can occur at any stage. “Women experience their healthcare across clinical specialties, but it often is fragmented or disconnected,” said Sadiya S. Khan, chair of the collaborative writing group. The publications connect those dots so that care is holistic and begins in adolescence. A 2026 American Heart Association scientific statement projected that if trends continue, high blood pressure among U.S. women 20 and older will rise from about 49% in 2020 to 59% in 2050, while maternal mortality has increased and cardiovascular disease now accounts for more than one-third of pregnancy-related deaths.

The framework also redefines menstrual cycle patterns as a female-specific vital sign and identifies Perimenopause as a cardiovascular inflection point. The 2026 American Heart Association/American College of Cardiology cholesterol guideline now includes pregnancy-related factors in risk assessment, and clinicians can use the PREVENT equations for 10- and 30-year sex-specific risk estimation. The three manuscripts include a A Female-Specific Life Course Approach to the Prevention and Management of Cardiovascular Disease, a Consensus Guidance for the Female-Specific Prevention and Management of Cardiovascular Disease and a Comprehensive Centers of Excellence for Women's Cardiovascular Care. The societies are calling for a national infrastructure built on interdisciplinary clinical care, professional education, research and community engagement. “All girls and women deserve more healthy years to live their fullest lives,” Khan said.
