The American Heart Association is alerting the public that the convergence of wildfire smoke drifting from Canada and record summer temperatures across parts of the United States creates a potentially deadly situation for heart health. The organization emphasizes that both extreme heat and air pollution from wildfires pose serious risks, particularly for individuals with existing cardiovascular conditions, and that the combined effect is even more hazardous.
“Extreme heat and wildfire smoke each carry major risks, especially for people with chronic health conditions including heart disease. The combination of the two is extremely dangerous,” said Manesh R. Patel, M.D., FAHA, volunteer president of the American Heart Association. “We want people to be mindful of the increased risk and take steps to protect themselves and their loved ones.”
While respiratory issues are commonly associated with wildfire smoke, Patel stressed that cardiovascular health impacts are significant and far-reaching. Wildfire smoke contains fine particulate matter that can enter the bloodstream and trigger inflammation, increasing the risk of heart attacks, strokes, and other cardiovascular events. A study published in Circulation, the American Heart Association’s flagship journal, found that the risk of fatal heart attacks nearly doubled during days with extreme heat and high levels of fine particle pollution, with risks even greater for women and older individuals.
Research has linked wildfire smoke exposure to a higher risk of sudden cardiac arrest and increased emergency room visits for cardiovascular issues. A study in the Journal of the American Heart Association reported that exposure to heavy smoke during wildfires raised the risk of out-of-hospital cardiac arrests by up to 70%, affecting both men and women, adults aged 35-64, and communities with lower socioeconomic status.
To reduce risks, the American Heart Association recommends staying indoors with doors and windows closed, using high-efficiency air filters, and limiting outdoor activity, especially during peak heat hours. Wearing a mask outdoors, staying hydrated with water, and avoiding alcohol and caffeine are also advised. Checking on older adults and those with chronic conditions is crucial, as they are more vulnerable.
Recognizing signs of a heart attack or stroke—such as chest discomfort, shortness of breath, or pain in the upper body—and calling 911 immediately can save lives. For heat stroke, symptoms include a body temperature above 103°F, hot red skin, and confusion. The association also highlights the importance of knowing CPR, which can double survival chances during cardiac arrest.
The U.S. Environmental Protection Agency offers a zip code-level air quality tracking map at airnow.gov. Patel advises checking it regularly and staying informed about local health alerts. “Protect yourself, be alert and prepared,” he said. “It could be the difference in saving your life or that of someone you love.”
